      Abstract
      Treatment for type 2 diabetes melittus (DM) needs a long-term treatment. The long-term treatment makes it possible for actual drug related problems (DRPs) to occur because of some factors like unnecessary drug therapy, wrong drug, need drug, too high dose, too low dose, non-adherences and drug interaction. Medication reconciliation is a process to obtain the accurate and complete patient’s medication list. The aim of this research is to identify and overcome actual DRPs in type 2 DM patients of Primary Health Center of Sewon 1 Bantul based on medication reconciliation result. This research is a non-experimental research with descriptive design. Data of type 2 DM patients of Primary Health Center of Sewon 1 Bantul from October to December 2015 was collected through a prospective approach by medication reconciliation process. Sample in this research consist of 35 type 2 DM patients and was taken by consecutive sampling technique. Actual DRPs were analyzed based on classification of DRPs using reference books and journal. The result showed that the use of drugs in type 2 diabetes patients were dominated by 3-drug combination with percentage of 42,86%. The use of Complementary and Alternative Medicine (CAM) consisted of herbal product in 3 patients (75%) and suplement in 1 patient (25%). The result of actual DRPs showed that 40% patients had actual DRPs. The actual DRPs could be classified as non-adherences (50%) and adverse drug reaction (50%). The actual DRPs could be overcome by providing counseling, antihyperglycemic drug substitution or recommend therapy to overcome the side effects of drugs.
