      Abstract
      Effective communication is one way to achieve the appropriate patient safety in patient safety standards. It is included in the cause of medical errors is ineffective communication. Efforts should be made to reduce the number of patient safety incidents in support of the safety of patients / patient safety can be done with effective communication strategic use of standard methods of S-BAR communication during the handoff. This research aims to determine the implementation of effective communication S-BAR methods on handoff in PKU Muhammadiyah hospital Yogyakarta Unit II. This research is descriptive quantitative analytical evaluative kind implementation of the Standard Operating Procedure. Respondents in this research were 85 people, the sample collection technique accidental sampling. the Instrument used was an observation checklist effective communication with SBAR method when operands case. These results indicate that at the point 1 to 3 said to correspond to a percentage of 100%, point 4 is 96.7%, point 5 to 6 is 100%, point 7 is 80%, point 8 is 100%, point 9 is 100% , point 10 is 96.7% , point 11 is 93.3%, point 12 is 100%, point 13 is 90%, point 14 is 93.3%, point 15 to 17 is 96.7%, point 18 is 23.3%, point 19 is 56.7%, point 20 is 6.7%, Data analysis that performed univariate analysis expressed with a frequency, percentage, accompanied by an explanation of the word, and measured by indicators: appropriate with the observation scores 76% - 100%, not appropriate with the observation scores <76%, thus can be concluded that there is most of the implementation of effective communication using S-BAR on handoff accordingly based on each item Standard Operating Procedure effective communication handover.
