      Abstract
      Diabetes mellitus (DM) is one of the highest prevalence diseases in the world, as well as in Indonesia which is ranked 5th with the highest number of DM patients in the world. Comprehensive service is necessary for the sufferer, home pharmacy care can be one alternative in DM disease control. The purpose is to determine the effect of providing home pharmacy care to changes in quality of life and Blood Sugar Levels (GDS) of patients DM type 2. The study included a quasi-experimental design with pretest and posttest with control group design. Respondents were patients with diabetes mellitus type 2 in Puskesmas Gamping I, as many as 30 respondents divided into 2 groups, the control group and the treatment group. The provision of home pharmacy care is done by using media leaflets given every 2 weeks for 2 months. The quality of life was measured using the questionnaire DQLCT (Diabetes Qualitu of Life Clinical Trial Quesionare) before and after the study, while the level of GDS was measured once every 1 month by 3 times the measurement. Sampling using simple random sampling technique. Data is processed using paired sample t-test and independent sample t-test. Home pharmacy care in patients diabetes type 2 in Puskesmas Gamping I, can increased the quality of life of the treatment group from 225.93 ± 23.2249 to 236.73 ± 23.801 with p value 0,000 which means there is a significant difference. Quality of life between groups does not show a significant difference with p value 0.294. GDS levels of the treatment group decreased from 266.40 ± 90.607 to 259.20 ± 35.368 with p value 0.444 it means there is not significant difference. Levels of intergroup GDS did not show any significant difference with p value >0,05.
