      Abstract
      Hypertension is one of the chronic metabolic diseases that have the risk of various complications. The complications can lead to decreased the quality of life. This study aims is to see the effect of home pharmacy care on the quality of life and outcome therapy of hypertensive patients. This is an experimental research. The observation was done prospectively with simple random sampling method. This study used 28 hypertensive patients in Gamping II Health Center which is divided into groups of control and intervention group. To measure the changes in blood pressure was used tensimeter and quality of life was measured using SF-36 questionnaire. The results showed that patients with hypertension given home pharmacy care in Gamping II Health Center had a significant decrease in blood pressure at the end of the study, systolic P = 0,003 (<0,05), diastolic P = 0,001 (<0,05) and quality of life respondents as measured with the SF-36 questionnaire showed significant improvement at all scales at the end of the study (P <0.05) in the intervention group over the control group. Implementation of home pharmacy care in hypertensive patients in Gamping II Health Center can improve the quality of life and also to lower systolic and diastolic blood pressure to reach the target blood pressure of hypertensive patients.
