      Abstract
      Background: The completeness of medical records is one of the good parameter in providing medical information. Increasing in hemodialysis treatment requires good initial assessment. The purpose of this study is to analyze the completeness of pre dialysis medical records at Clinic Hemodialysis of Health Center Nitipuran. Subjects and Method: Non experimental study using quantitative method. Research subjects are medical records of hemodialysis patients during august 2019, and medical personnel who involved in initial assessment. Descriptive data analysis of medical records and deep interview with doctor, nurse, and head nurse. Result: The completeness of pre dialysis medical record filling was found to be 100% in physical status, 7% in medical history, 42,7% in history of drug allergy, 21,9% in pain assessment, 92,7% in risk of falling, and 96,2% on education. Constraints faced include limited time for assess, assessment filled out without detailed inspection, there is a gap in the hourly monitoring record. Conclusion: The highest completeness is found in physical status, while the lowest is in medical history. Need additional time to assess or the use of additional tools such as electronic medical records.
