      Abstract
      Background: Infectious disease is one of the health problem in the world, including Indonesia. Infection can originate from the community or hospital environment (HAIs). HAIs that often occurs in hospital is Bloodstream Infection. The strategy that can control the number of HAIs is by surveillance. The purpose of this study was to determine the incidence of bloodstream infection at Nitipuran Health Center. Method: This research is a non-experimental analytic descriptive with cohort study approach. Quantitative samples were patient who do hemodialysis in total sampling (August 2019) and medical personnel directly involved in the hemodialysis process obtained from deep interview with purposive sampling technique. Result: The result showed that there were 4 patients who met the inclusion criteria. After undergoing a blood culture test, it was found that all four petients showed positive germs in the blood. The types of bacteria detected were Pseudomonas luteola, Staphyococcus koagulase negatif, Pseudomonas aeruginosa dan Burkholderia cepacia. This can arise due to lack of hand hygiene of medical personnel. Conclusion: four cases were identified using surveillance at Nitipuran Health Center. This can be caused by the awareness of medical personnel to maintain personal hygiene is still lacking and the compliance of medical personnel to conduct SOP of PPI is still low. The effort to overcome these problems is the formation of a PPI team and socialization of the PPI program.
