      Abstract
      Background : Chronic disease is a serious health problem and the biggest cause of death in the world. The 2013 WHO Global Report on Non Communicable Disease (NCD) data states that diabetes mellitus and hypertension are part of the main causes of death. People with chronic illness experience sudden changes in their lives that can cause a psychological reaction in the form of increased anxiety. In 2011 the Ministry of Health said there were 11.6% of anxiety and depression disorders in Indonesia. Seeing the relationship between anxiety and deterioration in chronic diseases, researchers are interested in conducting research in controlling and improving anxiety levels through client-centered counseling. This counseling uses the counselor's technique as a driver and the one who plays the role is the patient himself, patients are left to find their own solutions to the problem. Methods : The design of this study was quasi-experimental with a control group with pre-test and post-test design approach to determine the improvement of the patient's anxiety level in the treatment group and the difference in results with the control group. Subjects were 30 people with a diagnosis of type 2 diabetes mellitus and/or hypertension at Puskesmas Gedong Tengen and Puskesmas Kasihan 2 Yogyakarta. The study was conducted in November 2017 - May 2018. All subjects were divided into two groups, namely the treatment group which was given an intervention in the form of counseling and a control group that was not given intervention. Each group contains 15 people. Retrieval of data using primary data, namely the provision of questionnaires to respondents and secondary data medical records. Result : Statistical analysis p = 0.01 (p <0.05) using parametric test results with Paired Sample T-Test test there is a change in anxiety score and Independent sample T-Test test there is a significant difference between the two groups. Conclution : Client-centered counseling in the treatment group gave significant results in improving anxiety levels compared to the control group.
