      Abstract
      Discharge planning is a planning done for the patient and family before the patient leaves the hospital. Complications or failure to provide discharge planning will be at risk of disease severity. The determine whether overview of the implementation of the discharge planning to postsurgery patients of appendectomy in PKU Muhammadiyah Gamping Yogyakarta Hospital. Methods: The research used observational descriptive. The sample in this study 30 respondents implementation of discharge planning was done by the nurses in undergoing post-surgery patients of appendectomy in PKU Muhammadiyah Gamping Hospital. Result: The results of this study indicate that discharge planning actions at the beginning of the current assessment of patients admitted to the hospital there were 7 respondents (23.34%) did discharge planning and 23 respondents (76.66%) did not, the implementation of discharge planning for hospitalized patient, five respondents (16.66%) did the discharge planning and 25 respondents (83.34%) did not, and the implementation of discharge planning preparation for the repatriation, 12 respondents (40%) did the discharge planning and 18 respondents (60%) did not. Conclusion: Based on the result the Implementation of discharge planning in postsurgery patients of appendectomy at PKU Muhammadiyah Gamping Hospital, is not optimal because the nurses not really details with the discharge planning.
