| dc.description | Latar Belakang: Resistensi menjadi masalah kesehatan yang mendunia dengan dampak meningkatnya morbiditas, mortalitas, dan biaya kesehatan. Penggunaan antibiotik perioperatif dengan kecenderungan berlebihan menjadi salah satu penyebab resistensi. Perlu dilakukan evaluasi penggunaan antibiotik secara kuantitas dan kualitas. Tujuan penelitian ini adalah mengevaluasi penggunaan antibiotik pada pasien perioperatif yang dilakukan operasi bedah umum di Ruang Teratai RSUD dr. Soedirman Kebumen.
Subjek dan Metode: Penelitian menggunakan metode campuran (mixed method), dengan desain sequential explanatory, terhadap rekam medis pasien BPJS 3 dengan sampel 280. Subyek dalam penelitian ini adalah semua dokter spesialis Bedah Umum tetap, Apoteker, serta manajemen rumah sakit. Pengambilan data dilakukan secara retrospektif terhadap rekam medik dan wawancara. Analisis secara kuantitatif menggunakan metode DDD 100 patient days dan secara kualitatif menggunakan kriteria gyssens.
Hasil: Hasil evaluasi kuantitas penggunaan antibiotik di RS tersebut menunjukkan total DDD/100 patient-days sebesar 26,49, tertinggi pada ceftriaxone. Hasil evaluasi kualitas penggunaan antibiotik menunjukkan kategori 0 (tepat) 161 orang (57,5%), sisanya 22 orang (7,9%) kategori IVd (tidak tepat) dan 97 orang (34,6%) kategori V (tanpa indikasi). Sedangkan biaya penggunaan antibiotik Rp.3.774.776,88 dengan klaim BPJS Rp.4.068.122,50.
Kesimpulan : Para dokter cenderung meresepkan antibiotik dari golongan sefalosporin generasi ketiga (ceftriaxone). Belum semua dokter patuh terhadap regulasi penggunaan antibiotik yang ada di rumah sakit. Sedangkan biaya perawatan dapat diturunkan bila ketepatan kualitas penggunaan antibiotik ditingkatkan. | en_US |
| dc.description.abstract | Background: Resistance becomes a global health problem with the impact of increasing morbidity, mortality, and health costs. The use of perioperative antibiotics with excessive tendency is one of the causes of resistance. It is necessary to evaluate the use of antibiotics in quantity and quality. The purpose of this study was to evaluate the use of antibiotics in perioperative patients who performed general surgical operations in the Teratai Room of dr. Sudirman Hospital Kebumen.
Subjects and Methods: The study used a mixed method, with a sequential explanatory design, on the medical records of BPJS 3 patients with a sample of 280. The subjects in this study were all specialists in permanent General Surgery, Pharmacists, and hospital management. Data was collected retrospectively on medical records and interviews. Quantitative analysis using the DDD 100 patient days method and qualitatively using the Gyssens criteria.
Results: The evaluation results of the quantity of antibiotic use in the hospital showed a total DDD / 100 patient-days of 26.49, the highest in ceftriaxone. The results of evaluating the quality of antibiotic use showed category 0 (right) 161 people (57.5%), the remaining 22 people (7.9%) category IVd (incorrect) and 97 people (34.6%) category V (without indication) . While the cost of using antibiotics Rp. 3774,776.88 with BPJS claims Rp.4,068,122.50.
Conclusion: Doctors tend to prescribe antibiotics from the third generation cephalosporin (ceftriaxone). Not all doctors comply with the regulation of antibiotic use in hospitals. While the cost of treatment can be reduced if the quality of antibiotic use is improved. | en_US |
