IDENTIFIKASI DRUG RELATED PROBLEMS (DRPs) PADA PENATALAKSANAAN PASIEN CONGESTIVE HEART FAILURE (CHF) DI INSTALASI RAWAT INAP RS PKU MUHAMMADIYAH GAMPING PERIODE JANUARI-JUNI 2015
Abstract
Congestive heart failure (CHF) is a clinical syndrome that signed by heart structural and functional disorder which cause dyspnea and fatigue when doing activity. The pharmacological therapy of CHF can caused DRP are possible to occur drug related problems (DRPs). There are some types of DRPs such as adverse drug reaction, drug choice problem, dosing problem, drug use problem and drug interaction. The aim of this research is to describe the incidence of DRPs in the management oh hospitalized patients with CHF in RS PKU Muhammadiyah Gamping.
This research was a non-experimental research with cross-sectional descriptive analysis. Data was collected retrospectively from September 2015 to April 2016 from inpatients medical records. Sample in this research consist of 35 CHF inpatients which included in inclusion criteria. DRPs was analyzed based on classification of DRPS and use primary reference is Pharmaceutical Care Network Europe (PCNE) 2006, ACCF/AHA Guideline for The Management of Chronic of Cardiology Foundation/ American Heart Association Task Force on Practice tahun 2013 dan Pharmacotherapy: A Pathophysiologic Approach, 9th Edition.
The result showed that DRPs in the management of patients with CHF inpatients RS PKU Muhammadiyah Gamping indicates that are 20 patients (57,14%) it contains drug interaction with 35 incidence (77,78%), drug use problems with 6 incidence (22,22%), and not found in adverse drug reaction, dosing problems and drug choice problems category
