Accreditation is the assessment feasibility program in an education unit based on the criteria that has been set. The university accreditation is implemented within 5 years. In implementing the whole accreditation process in information technology UMY, it must prepare borang to fulfill all the aspects which has been measurement in evaluating and assessing the quality of information technology program. That borang is the data for 4 years before accreditation. The process of borang archive accreditation storage in information technology UMY uses unsystematic wardrobe and file explorer that no password or security so that it is risky from illegal action of the unwanted and dropbox. The problem can be reduced by making an application capable to manage borang archive accreditation in digital form. The method used in making application is waterfall. Application desktop is created by using a java programming language and mysql database. Based on testing result, it can be concluded the application can manage borang archive accreditation so that it can store data in digital form and systematize with security system.
