This study aims to analyze how the implementation of product quality control in CV. Yogyakartas and analyze the factors causing the product damage. Sample in this research is dus product or paper box taken as many as 36 sample. The analysis is done by collecting the production data and product damage data presented in the form of check sheet, then processing the data of production quantity and the amount of damage box by using the analysis tool Controlling the quality of control chart P, and analyzing the cause of damage using cause and effect diagram. Based on the result of statistical quality control analysis, the result of 4 (four) production process process still there are some samples that pass the limit of damage tolerance, including 8 samples in print process, 7 samples in lamination process, 4 samples in pond process and 5 samples on the gluing process. Based on the results of the analysis of the causes of damage is known that the most influential cause of damage from 4 (four) stage of the production process is human, machine and method.
