This study aims to describe the model of supply chain scheme, describe the role of stakeholders, identify problems faced by each stakeholders and solutions to solve the problem. The research was conducted in Manggoro village, Tembarak, Temanggung, Central Java. The subjects of this research are farmers, collectors, factories, distributors, consumers of Corn Chips. Sampling technique in this research by purposive sampling technique with criteria that know about supply chain of Corn Chips by interviewing 14 respondents. Data analysis techniques in this study using content analysis. The results of this study indicate that the Corn Supply Chain network starts from the farmer who sells corn to the collectors, the collectors sell the corn to the factory. The factory markets its products to consumers through distributors. The role of stakeholders in the supply chain of Corn Chips is how to pay attention to the sustanbility of the corn production process to become the Corn Chips according to the quality standard. In this research also obtained identification problem and solution faced by every stakeholders.
