      Abstract
In the procedure surgery, infection is possible in patients who are hospitalized. That Infection often occurs in post-surgical patients is surgical site infection (SSI) (Pandjaitan, 2013). Therefore, it is necessary to use prophylactic antibiotiks in hospitals as a preventive effort to prevent the occurrence of surgical area infections. Antibiotiks should be administreted on a rational base due to the growing bacterial resistance (Gyssens, 1999). So, the WHO and Europian Commission recommends to study rational antibiotik use. The period with the assessment of data on the exact use of indications, exact medication, condition of patients according to this study is to determine the accuracy and prophylactic antibiotik use in patients with urological surgery at PKU Yogyakarta Hospital. to the European Association of Urology (EAU) (2015), and appropriate dose according to PKU Muhammadiyah Yogyakarta Hospital Formulary (2015). This research is a descriptive observational study uses cross sectional design with retrospective data retrieval and purposive sampling sampling technique. Data retrieval uses secondary data in the form of medical record records. Then the picture and appropriateness data with guidelines are analyzed in the form of percentages. In this study, a description of prophylactic antibiotiks was obtained including ceftriaxone (89%), cefotaxime (10%), levofloxacin (1%), administration time 1-30 minutes (44%), route of administration (100%) intravenously, and the duration of administration is given a single dose of ≤24 hours (100%). The rationality of prophylactic antibiotik use according to EAU in 2015 is inappropriate indication (24%), inappropriate drug (0%), according to the RS Formulary the dose used is 100% correct, and the clinical condition of all patients (100%) is considered appropriate
