| dc.description | Latar Belakang: Sejak tahun 2014 Indonesia telah menerapkan Jaminan
Kesehatan Nasional sebagai asuransi sosial. Skema pembayaran JKN bukan
lagi fee for service melainkan berdasarkan tarif Ina CBG‘s. Dengan skema
tersebut, tarif pembayaran untuk pasien DM disamaratakan (Rp 185.000 per
kunjungan) meskipun terdapat komplikasi. Padahal pasien DM dengan
komplikasi diperkirakan membutuhkan biaya lebih tinggi. Penelitian ini
bertujuan untuk mengetahui hubungan antara jenis dan jumlah komplikasi
DM dengan baya perawatan.
Metode: Penelitian dilakukan secara analitik cross sectional. Data sekunder
diambil dari rekam medis serta rincian biaya perawatan pasien DM JKN Non
PBI di Instalasi Rawat Jalan Penyakit Dalam RSI Jombang periode 1 Januari
2014-31 Desember 2016 dengan metode total sampling. Sebanyak 626
sampel memenuhi kriteria inklusi dan eksklusi. Hasil penelitian dianalisa
dengan uji non-parametrik.
Hasil: Ada korelasi yang signifikan antara komplikasi dan kunjungan ke
rumah sakit dalam setahun (p = 0,000). Rata-rata biaya pengobatan per
kunjungan tertinggi ditemukan pada DM tanpa komplikasi (Rp 154.626)
sedangkan yang terendah ditemukan pada DM dengan komplikasi
mikrovaskuler dan makrovaskuler (Rp 123.664). Ada korelasi negatif dan
signifikan antara jenis dan jumlah komplikasi DM dan biaya medis (r = -
0.199, r = -0.247, p = 0,000). Kedua biaya medis masih dalam klaim Ina
CBG.
Kesimpulan: Pasien DM tipe 2 membutuhkan biaya tinggi akan tetapi rumah
sakit mampu melakukan kendali biaya. Adanya hubungan negatif antara jenis
dan jumlah komplikasi dengan biaya medis dapat berpengaruh kepada
kualitas pelayanan.

| dc.description.abstract | Background: Since 2014 Indonesia has implemented National Health
Insurance as a social insurance. The JKN payment scheme is no longer a fee
for service but use Ina CBG's tariff scheme. Under the Ina CBG‘s, the
payment rate for DM type 2 patients is generalized (Rp 185,000 per visit)
although DM patients with complications required higher cost. This study is
intended to determine the relationship between DM complications with
medical cost.
Method: The research was done analytically and cross sectional. Secondary
data is taken from medical record and details of patient care cost of DM JKN
Non PBI at Outpatient Installation of Internal Disease RSI Jombang period 1
January 2014 to 31 December 2016 by total sampling method. A total of 626
samples met inclusion and exclusion criteria. The results were analyzed by
non parametric test..
Result: There was a significant correlation between complication and and
hospital visit in a year (p = 0.000). The highest average medical cost per visit
was found in DM without complication (Rp 154,626) while the lowest was
found in DM with both microvascular and macrovascular complication (Rp
123,664). There is negative and signifficant correlation between type and
number of DM complication and medical cost (r = -0.199, r = -0.247, p =
0.000). Both medical cost still under Ina CBG‘s claim.
Conclusion: DM type 2 with complication needs higher cost but hospital was
able to manage the cost efficiently. But this may affect the quality of
treatment. Government, hospital and physician must take further
consideration.
