      Abstract
      In Indonesia, cardiovascular disease is ranked first as the cause of death, with 17.5 million people die each year. In RSUD of Yogyakarta the incidence rate of Congestive Heart Failure (CHF) is 0.8% from the total number of patients in the inpatient installation. Cardiovascular disease is generally accompanied by comorbidities so that patients get complex therapy. The importance of pharmacy care in the form of actual or potential Drug Related Problems (DRPs) is intended to achieve optimal therapy. This study aims to determine the categories and incidence rates of DRPs in CHF patients in the Inpatient Installation in RSUD of Yogyakarta for the period of January-December 2016. This study used a non-experimental research design that was analyzed descriptively. Data collection of CHF patients in RSUD of Yogyakarta was conducted retrospectively starting from October to December 2017. The sample of the study consisted of 36 patients with a diagnosis of CHF that met the inclusion criteria. DRPs analysis in this study used the Medical Service Standards (SPM) of RSUD of Yogyakarta, Guideline Pharmacotherapy Handbook 9th edition 2015, NYHA, Drug Interaction Facts and PERKI. The results of the analysis of the identification of DRPs show that there are 19 patients (52%) who had the potential to experience DRPs as many as 42 incidents including the problem of drug selection as many as 20 incidents (48%), the problem of giving dose as many as 1 incident (2%) and drug interactions as many as 21 incidents (50%).
