PERBANDINGAN KEEFEKTIFAN KOMBINASI TERAPI NATRIUM DIKLOFENAK TOPIKAL 0,1% DAN TIMOLOL MALEAT 0,5% DENGAN MONOTERAPI NATRIUM DIKLOFENAK TOPIKAL 0,1% UNTUK MENGURANGI RASA NYERI PADA PASIEN PASCA OPERASI FAKOEMULSIFIKASI
Abstract
Background: One of the cataract management is the operation of
phacoemulsification method. However, post-surgery phacoemulsification can
cause mechanical trauma, changes in eye structure, inflammation in the
trabeculum, and the possibility of residual cortex or cohesive dispersive
viscoelastic material is left behind. This makes the remaining cortex undergo the
process of phagocytosis by macrophages and the remaining phagocytic tissue will
accumulate in the trabeculum thereby increasing intraocular pressure. Increased
intraocular pressure will damage the lamina kribosa and nerve axons, suppress
the optic nerve, and can make further optic nerve damage. These things can cause
pain in the eyes.
Purpose: To determine the difference in effectiveness of the therapy combination
of topical sodium diclofenac 0.1% and 0.5% timolol maleate compared with
topical diclofenac sodium monotherapy 0.1% to reduce pain measured using the
Visual Analog Scale in post phacoemulsification surgery cataract patients.
Method: The method of this research is experimental analytic with Single Blind -
Randomized Clinical Trial design.
Results: On the 1st day the p value = 0.759 (p> 0.05), on the 7th day the p value
= 0.066 (p> 0.05) and the average of days 1 and 7 the p value = 0.306 (p> 0) ,
05). The average preoperative intraocular pressure in the monotherapy group
was 19.90 ± 4.01 mmHg and the combination group was 19.31 ± 4.38 mmHg, and
p value = 0.581 (p> 0.05) was obtained. At preoperative stage it cannot be
determined whether or not there has been a decrease in intraocular pressure. The
average postoperative intraocular pressure in the monotherapy group was 16.67
± 3.85 mmHg, the combination group was 16.80 ± 3.93 mmHg, and the p value =
0.877 (p> 0.05).
Conclusion: There was no significant difference in pain (p> 0.05) on days 1 and
7 after phacoemulsification cataract surgery in PKU Muhammadiyah Yogyakarta
Hospital
