IDENTIFIKASI DRUG RELATED PROBLEM POTENSIAL MELALUI PROSES REKONSILIASI OBAT PADA PASIEN DIABETES MELITUS TIPE 2 DI PUSKESMAS SEWON 1 BANTUL
Abstract
Treatment for diabetes mellitus type 2 are modifying the lifestyle and pharmacological therapy. DRPs are possible to occur through pharmacological therapy because of various factors such as gender, age, the number of drugs and less of patient’s education. Medication reconciliation is a process to obtain the accurate and complete patient’s medication list. The aim of this research is to describe the prevalence and evaluate potential DRPs in DM type 2 of Primary Health Center of Sewon 1 Bantul through medication reconciliation.
This research was a non-experimental research with descriptive analysis. Data of DM type 2 patients of Public Health Center of Sewon 1 Bantul from August to December 2015 was collected through a cross sectional approach by medication reconciliation process. Sample in this research consist of 35 DM type 2 patients and was taken by consecutive sampling technique. Potential DRPs were analyzed based on classification of DRPs and conducted by reference books and journal.
The result showed that the use of drugs in type 2 diabetes patients were dominated by 3-drugs combination with percentage of 42,86%. The use of Complementary and Alternative Medicine (CAM) consisted of herbal product in 3 patients (75%) and alternative therapy in 1 patient (25%). The result of potential DRPs showed that 29 patients had potential DRPs with percentage of 82,86%. DRPs found could be classified on non-adherences (44,82%), adverse drug reaction (20,7%) and potential drug interactions (34,48%).
