Penyuluh Keluarga Berencana (PKB, Family Planning Counselor) is a field frontline worker of the family planning program. Undang-undang Republik Indonesia (Law of the Republic of Indonesia) Number 52 Year 2009 about The Population Growth and Family Development and Law Number 62 Year 2010 about Badan Kependudukan dan Keluarga Berencana Nasional (BKKBN; Population and Family Planning National Agency) state that BKKBN has the duty to implement government duty of population growth control and family planning. However, implementation of that role face obstacles. Aims of this study are to identify the role of Family Planning Counselor in reproductive health promotion to adolescents in Pedukuhan (Hamlet of) Kidul Nyamplung, Desa (Village of) Balecatur, Kecamatan (District of) Gamping, Yogyakarta; and determine supporting factors and barriers of implementation of that role. The research method is qualitative research method with the case study. Techniques of data collecting interview, observation, and documentation. The results of the study show that: (1) the role of family extensionists in improving health by making programs as attractive as possible, sports missal programs, counseling about drugs, AIDS, HIV and special counseling for adolescents. (2) the supporting factors of each PKB program. Good cooperation with the community layer as for the inhibiting factors of PKB in running the program from schools in schools or campus is difficult to collect adolescents from inhibiting factors PKB is less complete facilities and infrastructure such as buildings that have not become people who have to work indoors that is sober.
